The next step in your journey is to determine if your loved one physically qualifies for skilled nursing care. This can be determined by your doctor who can prescribe skilled nursing care on a special form #3008…which will be required by DCF at the time of your application.
There are 10 “Activities of Daily Living”. They are: Walking, Transferring (getting in and out of bed or chair), toileting, dressing, bathing, eating, administering medications, cognitive impairment, vision and/or hearing issues. These are an assessment that your doctor will use, among other criteria in determining the necessity for 24/7 custodial care for your loved one. Your loved one must be deficient in at least three areas. By reviewing this list, you might be able to determine whether your loved one’s doctor may prescribe full time care.
When you meet with your loved one’s doctor, request that he/she complete a #3008 form and return it to you. It may take a little while for the doctor and his office to complete this form, therefore, it is important that you request it as soon as you have determined that your loved one may require 24/7 custodial care.
